We will file insurance on your behalf, if you have provided us with a clear
copy of your insurance card. If you are not covered by insurance, we ask that
you pay, in full, at the time services are rendered.
Except in certain circumstances, your health insurance is a contract between
you, the provider and your insurance company. We ask that you not lose sight
of this fact. It is, therefore, necessary that you take the responsibility
to follow-up on a claim to your company if you feel that it is not being paid
promptly or properly. We will be happy to provide you with any information
we can assist you with your claim.
In order for us to file an insurance claim, it is necessary we be provided
with all pertinent information. Your insurance card will be copied when you
arrive at our office. It is important to have your insurance card with you
on the day of your visit. Otherwise, we must collect full payment at the time
of your visit.
Should you have financial problems, which delay payment of your bill, please
contact us. We won’t know you are having problems, unless you tell us.
We will make every effort to work out an acceptable monthly payment plan to
enable you to take care of your obligations.
For your convenience, we accept VISA and MASTERCARD for payment of services
not covered by your insurance.
