We will file insurance on your behalf, if you have provided us with a clear copy of your insurance card. If you are not covered by insurance, we ask that you pay, in full, at the time services are rendered.
Except in certain circumstances, your health insurance is a contract between you, the provider and your insurance company. We ask that you not lose sight of this fact. It is, therefore, necessary that you take responsibility for follow up on a claim that is made to your insurance company if you feel that it is not being paid promptly or properly. We will be happy to provide you with any information needed to assist you with your claim.
In order for us to file an insurance claim, we will need to make a copy of your insurance card when you arrive at out office. It is important to have your insurance card with you on the day of your visit. If you do not have your insurance card, we must collect full payment at the time of your visit.
Should you have financial problems, which delay payment of your bill, please contact us. We will make every effort to work out an acceptable monthly payment plan to enable you to take care of your obligations.
The forms of payment accepted by this office include CASH, CHECK, VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS, DEBIT CARDS, MONEY ORDERS and CASHIERS CHECKS for payment of any services not covered by your insurance.
We also offer secure online bill payment using PayPal
